Food Focus Group Consent and Agreement

By clicking submit, I acknowledge that I have read, understood, and agreed to the terms and conditions outlined in this Consent and Agreement form. I agree to the following terms and conditions to participate in the food focus group organized by Kave Brothers Foods:

Consent to Participate:

1. I voluntarily agree to participate in this food focus group and provide my honest feedback, opinions, and evaluations of the products presented.
2. I understand that my participation is voluntary, and I may withdraw from the focus group at any time without consequence.
3. I confirm that I meet all the eligibility criteria specified for this focus group, including being over the age of 18 and having no food allergies or dietary restrictions that would prevent me from sampling the products.

Confidentiality and Non-Disclosure:

1. I agree to maintain strict confidentiality in perpetuity regarding all information, products, and discussions related to any focus group or surveys.
2. I will not disclose any proprietary information, trade secrets, or confidential details about the products, recipes, or formulations to any third party, including but not limited to competitors, media, or social media platforms.
3. I understand that any unauthorized disclosure or misuse of confidential information may result in legal action being taken against me.

Honest Feedback and Opinions:

1. I agree to provide honest, unbiased, and constructive feedback and opinions during the focus group sessions.
2. I will not intentionally misrepresent my thoughts or experiences with the products being evaluated.
3. I understand that my feedback and opinions will be used to improve and develop better products, and I will strive to provide valuable insights to the best of my ability.
